Diabetes Self-Management Program at Alexian Brothers Nutrition Services
Alexian Brother’s Health System features a Comprehensive Diabetes Self-Management Program, recognized by the American Diabetes Association.
We offer individual and group education sessions with Registered Dietitians (RD) and Registered Nurses (RN). Individual sessions with a RD will focus on designing a customized meal plan focusing on foods containing carbohydrates and how they affect blood sugar. Individual sessions with a RN will focus on how to check and manage blood sugars and medication management.
Our group classes offer comprehensive information on important aspects of diabetes management. Our program is structured as a series of 4 classes, each class is 2 hours in length. Classes are taught by our Certified Diabetes Educators (CDE). CDE’s are nurses and dietitians who are specialist in diabetes-related education who are equipped to provide extensive self-management training to those with diabetes and their families to improve health and quality of life. Topics covered include: basic understanding of diabetes, blood glucose monitoring, medication management, meal planning and behavior management.
We also offer individual and group sessions for Gestational Diabetes.
